背景技術(shù):
疼痛根據(jù)其神經(jīng)生理學(xué)機(jī)制可分為感受性疼痛和神經(jīng)病理性疼痛兩類,前者直接由傷害性刺激引起,與組織損傷或炎癥反應(yīng)有關(guān),又稱炎癥性疼痛;后者由軀體感覺神經(jīng)系統(tǒng)的損傷或疾病直接造成的慢性疼痛。
神經(jīng)生長因子(nervegrowthfactor,ngf)是意大利科學(xué)家levi-montlcini于1953年在小鼠肉瘤細(xì)胞內(nèi)發(fā)現(xiàn)的第一個(gè)神經(jīng)營養(yǎng)因子,ngf是一種具有神經(jīng)元營養(yǎng)和促進(jìn)突起生長雙重生物學(xué)功能的一種神經(jīng)細(xì)胞生長調(diào)節(jié)因子,它對中樞及周圍神經(jīng)元的發(fā)育、分化、生長、再生和功能特性的表達(dá)均具有重要的調(diào)控作用。ngf包含α、β、γ三個(gè)亞單位,β亞單位是活性區(qū),由兩條單鏈通過非共價(jià)鍵結(jié)合而成。levi-montalcini因?yàn)榘l(fā)現(xiàn)了ngf而獲得了諾貝爾獎(jiǎng)。目前,國內(nèi)外已有多個(gè)ngf產(chǎn)品上市,臨床上主要用于治療神經(jīng)系統(tǒng)發(fā)育不良,包括弱視、神經(jīng)瘤、各種神經(jīng)損傷及神經(jīng)系統(tǒng)病變等疾病。
ngf存在于多種物種中,在雄性小鼠頜下腺、牛精漿、蛇毒、豚鼠前列腺和人胎盤組織中含量豐富。其中小鼠ngf與人ngf氨基酸序列同源性達(dá)到90%。考慮到鼠ngf應(yīng)用于人體的種屬差異性及小鼠作為原材料所具有的潛在的致病因子風(fēng)險(xiǎn)及人胎盤組織原材料的限制,發(fā)展基因工程技術(shù)制備重組人ngf(rhngf,recombinanthumanngf)來取代提取的鼠ngf和人ngf具有很好的應(yīng)用前景。
體內(nèi)成熟的ngf以同源二聚體的形式存在,每條肽鏈包含120個(gè)氨基酸。人ngf基因位于1號染色體短臂上,完整的ngf外顯子由241個(gè)氨基酸組成,通常稱為preprongf前體,在內(nèi)質(zhì)網(wǎng)中preprongf前體的信號肽被切割下來,形成prongf前體(223個(gè)氨基酸),prongf前體在內(nèi)質(zhì)網(wǎng)中以同源二聚體形式存在,然后轉(zhuǎn)移至高爾基體中,前體部分經(jīng)furin酶切,形成成熟的ngf二聚體(單體含有120個(gè)氨基酸),被轉(zhuǎn)運(yùn)至細(xì)胞外,同時(shí)也有部分未經(jīng)切割的prongf前體分泌至細(xì)胞外。
重組人源ngf雖然避免了一些潛在的致病風(fēng)險(xiǎn),但在實(shí)際使用過程中仍然會(huì)存在很大問題:1)保持ngf生物活性,像其他蛋白質(zhì)一樣,ngf的生物活性依賴于其二級和三級結(jié)構(gòu),故在制備、純化、存儲(chǔ)及給藥期間過程中保持其生物活性特別重要;2)ngf使用過程中會(huì)引起較嚴(yán)重的疼痛,部分病人無法耐受,故造成了使用上的部分受限。ngf參與了疼痛的病理生理過程,通過影響炎癥介質(zhì)的釋放、離子通道的開放及促進(jìn)神經(jīng)纖維的生長導(dǎo)致疼痛,并通過調(diào)控離子通道及分子信號從而參與疼痛的發(fā)生發(fā)展。有學(xué)者推測ngf還可能通過促進(jìn)致痛物質(zhì)的表達(dá)導(dǎo)致疼痛,并在機(jī)體損傷后可改變神經(jīng)元的出芽和再生。目前的研究發(fā)現(xiàn):在人類中,不會(huì)引起痛覺過敏的最大劑量為0.03μg/kg(pettyetal.,1994--29)。但是,如此低的劑量限制了ngf的應(yīng)用,同時(shí)也限制了其適應(yīng)癥的擴(kuò)大,如用于中樞神經(jīng)系統(tǒng)。
因此,為避免上述問題,需尋求一種可以減輕疼痛副作用甚至無痛的重組hngf,從而可加大使用劑量及受試人群,為擴(kuò)大適應(yīng)癥以及應(yīng)用于中樞神經(jīng)系統(tǒng)提供了可能。
技術(shù)實(shí)現(xiàn)要素:
本發(fā)明的目的是提供一組具有低疼痛性甚至無痛的神經(jīng)生長因子突變體,即重組hngf。
為實(shí)現(xiàn)上述目的,本發(fā)明采用以下技術(shù)方案:
低痛神經(jīng)生長因子突變體,為序列表seqidno:3至seqidno:49中任意一個(gè)所示的氨基酸序列。
編碼所述的低痛神經(jīng)生長因子突變體的核苷酸序列。
所述的核苷酸序列,包括基因組dna、cdna、合成的dna和rna。優(yōu)選dna,更優(yōu)選為編碼序列的cdna。本發(fā)明還涵蓋編碼具有如本文所定義的特定性質(zhì)的酶或蛋白的核苷酸序列。本文所用的術(shù)語"核苷酸序列"是指寡核苷酸序列或多核苷酸序列和其變體、同源物、片段和衍生物(如其部分)。所述核苷酸序列可以是源自基因組或合成物或重組物,其可以是雙鏈的或單鏈的(無論其代表正義鏈還是反義鏈)核酸分子??梢酝ㄟ^成熟的標(biāo)準(zhǔn)方法通過合成來制備編碼所述低痛神經(jīng)生長因子突變體的核苷酸序列,利用自動(dòng)dna合成儀上合成寡核苷酸,然后將其擴(kuò)增、酶切連接至適當(dāng)?shù)妮d體中。
本發(fā)明所述低痛重組hngf突變體是在野生型hngf序列(seqidno:2)基礎(chǔ)上進(jìn)行單點(diǎn)或多點(diǎn)突變得到的,優(yōu)選以下ngf突變體,其氨基酸序列如下:

所述宿主細(xì)胞為哺乳動(dòng)物細(xì)胞、酵母細(xì)胞、昆蟲細(xì)胞、大腸桿菌、絲狀真菌或植物細(xì)胞。優(yōu)選地,所述的宿主細(xì)胞為哺乳動(dòng)物細(xì)胞。
所述哺乳動(dòng)物細(xì)胞為中國倉鼠卵巢細(xì)胞、人胚腎293細(xì)胞、cos細(xì)胞或hela細(xì)胞。所述昆蟲細(xì)胞為sf9細(xì)胞、sf21細(xì)胞、sf900細(xì)胞。
一種藥物組合物,所述藥物組合物含有藥學(xué)上可接受的賦形劑,以及上述提及的低痛神經(jīng)生長因子突變體、上述提及的表達(dá)載體和上述提及的宿主細(xì)胞中的一種或幾種。
本發(fā)明的藥物可以制成注射劑、膠囊、片劑或粉劑等多種形式,上述各種劑型的藥物均可以按照藥學(xué)領(lǐng)域的常規(guī)方法制備。
所述藥物組合物優(yōu)選為注射液,所述注射液包括藥學(xué)上可接受的賦形劑以及上述提及的低痛神經(jīng)生長因子突變體。
需要的時(shí)候,在上述藥物組合物中還可以加入一種或多種藥學(xué)上可接受的載體,所述載體包括藥學(xué)領(lǐng)域常規(guī)的稀釋劑、穩(wěn)定劑、表面活性劑和防腐劑等。

本發(fā)明的優(yōu)點(diǎn)在于:相對于現(xiàn)有技術(shù)中的野生型神經(jīng)生長因子而言,本公開的低痛神經(jīng)生長因子可以減輕疼痛副作用甚至無痛,可增加患者對神經(jīng)生長因子的依從性和耐受性,可加大使用劑量及受試人群,為擴(kuò)大適應(yīng)癥以及應(yīng)用于中樞神經(jīng)系統(tǒng)提供了可能。

具體實(shí)施方式
實(shí)施例1.野生型hngf及其突變體的質(zhì)粒構(gòu)建
1、構(gòu)建野生型hngf的dna序列表達(dá)質(zhì)粒
合成野生型hngf的dna序列(序列表seqidno:1),使用引物(f:ggaattcatgtccatgttg(序列表seqidno:50),r:caagctttcaggctcttct(序列表seqidno:51)對目的序列進(jìn)行pcr擴(kuò)增,使用ecori(neb#r0101s)進(jìn)行酶切后,將酶切產(chǎn)物使用hindiii(neb#r0104s)進(jìn)行二次酶切。將pcdna3.1(-)表達(dá)載體使用同樣的方法進(jìn)行酶切。將酶切載體和pcr擴(kuò)增目的片段進(jìn)行瓊脂糖凝膠電泳,切下目的片段,使用dna膠回收試劑盒(tiangen,#dp209-03)分別回收酶切后的載體和目的dna片段后,使用dna連接酶試劑盒(takara/6022)16℃連接1h,完成野生型hngf的質(zhì)粒構(gòu)建。
2、構(gòu)建hngf突變體的dna序列表達(dá)質(zhì)粒
同樣上述方法,我們分別合成了編碼序列表seqidno:3至seqidno:49所示的低痛神經(jīng)生長因子突變體的基因,并分別構(gòu)建了含有上述編碼基因的質(zhì)粒。
實(shí)施例2、hngf及其突變體的質(zhì)粒轉(zhuǎn)化及提取
1、轉(zhuǎn)化
將上述實(shí)施例1構(gòu)建好的hngf及其突變體質(zhì)粒進(jìn)行熱激轉(zhuǎn)化,將top10感受態(tài)細(xì)胞(天根/cb104-02)從-70℃冰箱取出后立即置于冰上解凍,取50ul用于轉(zhuǎn)化。向其中加入2ul質(zhì)粒,輕彈混勻,冰浴30min。于42℃干浴90s,期間不要振蕩離心管,取出后立即置于冰上2min。加入無抗lb/soc培養(yǎng)基500ul,150rpm/min,37℃搖床培養(yǎng)45min。將離心管內(nèi)液體全部倒入lb平板上,均勻涂開。待平板晾干后,倒置于培養(yǎng)箱培養(yǎng)16h。
2、質(zhì)粒大提
挑取上述轉(zhuǎn)化步驟得到的單菌落接種于500ullb液體培養(yǎng)基,37℃培養(yǎng)7h,同時(shí)菌液送檢測序。將測序正確的菌液進(jìn)行大量搖菌,將500ul菌液接種于500mllb培養(yǎng)基,37℃培養(yǎng)16h。4℃離心收集過夜培養(yǎng)的菌液,6000xg離心10分鐘,充分棄去上清使用質(zhì)粒大提試劑盒(購自qiagen,貨號12163)大提質(zhì)粒,測量濃度備用。
實(shí)施例3.野生型hngf及其突變體的表達(dá)
將上述實(shí)施例2大提得到的野生型hngf和及其突變體質(zhì)粒轉(zhuǎn)染293f細(xì)胞中,轉(zhuǎn)染后第四日收集表達(dá)上清并定量。
實(shí)驗(yàn)步驟:
1、轉(zhuǎn)染前一天,接種293f細(xì)胞,0.5×106/ml,共900ml,300ml/瓶。
2、轉(zhuǎn)染當(dāng)天計(jì)數(shù),細(xì)胞密度約1.0×106/ml,活率99%以上。
3、轉(zhuǎn)染:取36ml細(xì)胞培養(yǎng)基到125ml培養(yǎng)瓶;加360ug質(zhì)粒,混勻;再加1080ugpei,混勻。室溫靜置15分鐘;與細(xì)胞混勻,約12.3ml/瓶;37℃,8%co2,120rpm培養(yǎng)。
4、轉(zhuǎn)染后第四日收集細(xì)胞上清,10000g離心20分鐘。
5、收集上清,0.45um過濾,得到野生型hngf及其突變體的蛋白上清液。
6、sds-page檢測,硝酸銀染色定量。
實(shí)施例4.野生型hngf及其突變體的純化
純化上述實(shí)施例3中得到的ngf及其突變體的蛋白上清液。
1、陽離子交換層析:先使用乙酸和水調(diào)野生型hngf及其突變體的蛋白上清液至ph為4.0。cmsepharoseff層析柱用0.05mol/l乙酸鹽緩沖液(ph4.0)充分平衡后上樣,上樣完成后,用平衡液沖洗至基線,用0.05mol/ltris-hc1(ph9.0)緩沖液洗脫雜峰至基線,再用0.05mol/ltris-hc1和0.05mol/ltris-hc1-0.4mol/lnacl(ph9.0)梯度洗脫,根據(jù)紫外吸收情況收集目標(biāo)峰,在紫外檢測儀上顯示數(shù)字開始上升時(shí)開始收集,降為基線時(shí)停止收集目標(biāo)蛋白峰。
2、疏水層析:butylsepharose4ff層析柱經(jīng)0.02mol/l磷酸鹽(ph6.8)-1.5mol/l氯化鈉緩沖液充分平衡。在步驟1收集的目標(biāo)峰料液中加入氯化鈉固體使料液中氯化鈉的終濃度為1.5mol/l,待氯化鈉充分溶解后上樣,速度為120cm/h,上樣完成后用平衡液沖洗至基線,然后用0.02mol/l磷酸鹽(ph6.8)洗脫收集目標(biāo)峰。
3、凝膠排阻層析:superdex75prepgrade層析柱用ph6.8、0.05mol/l磷酸鹽-0.15mol/l氯化鈉緩沖液充分平衡后上樣步驟/2中收集的目標(biāo)峰,在紫外檢測儀上顯示數(shù)字從基線開始上升時(shí)開始收集,降為基線時(shí)停止收集目標(biāo)蛋白峰。
superdex75柱純化的野生型hngf的sdspage如圖1所示,說明制備得到的ngf純度較高。將收集的野生型hngf及其突變體目標(biāo)蛋白峰樣品使用超濾管濃縮至0.4mg/ml,4℃保存用于后續(xù)實(shí)驗(yàn)。
實(shí)施例5.雞胚法測定野生型hngf及其突變體活性
1、雞胚背根神經(jīng)節(jié)法測量野生型hngf及其突變體活性
將上述實(shí)施例4中得到的野生型hngf及其突變體樣品進(jìn)行稀釋:a液:6ng提取的野生型hngf及其突變體樣品加1ml無血清dmem培養(yǎng)液溶解;b液:取a液50μl加無血清dmem培養(yǎng)液4.95ml;c液:取b液60μl加無血清dmem培養(yǎng)液2.94ml(總量3ml)使終濃度為(3au/ml)。a、b液稀釋在離心管中,c液在細(xì)胞瓶內(nèi),將c液作為1號瓶,再3倍梯度稀釋為:2號、3號、4號、5號、6號待測液。每個(gè)待測液加入1個(gè)培養(yǎng)瓶,2ml/瓶。同時(shí)以無血清dmem培養(yǎng)液為空白對照,以購自中國食品藥品檢定研究院的標(biāo)準(zhǔn)品為陽性對照(參考品)。加入8日齡的雞胚背根神經(jīng)節(jié)后置于5%co2、37℃飽和濕度培養(yǎng)箱中,24小時(shí)后觀察結(jié)果。
以生長最好時(shí)每毫升待測樣品中ngf的含量作為1個(gè)活性單位(au)。從出現(xiàn)陰性對照結(jié)果的稀釋度開始往回?cái)?shù)第3和第4兩個(gè)稀釋度中取生長最好的作為判定終點(diǎn)計(jì)算效價(jià)。參考品為購自中檢院的標(biāo)準(zhǔn)品,每支裝量為1000au。
ngf比活的計(jì)算公式為:
待測樣品的比活(au/mg)=參考品活性(au/ml)×[樣品預(yù)稀釋倍數(shù)×對應(yīng)參考品稀釋點(diǎn)處的活性(au/ml)/參考品實(shí)測活性(au/ml)]
測活結(jié)果表明,低痛hngf突變體,均保留了野生型hngf的活性。
實(shí)施例6.ngf及其突變體是否致痛(疼痛閾值)檢測
實(shí)驗(yàn)原理:篩選對痛覺反應(yīng)正常的合格小鼠,注射一定劑量的ngf樣品(野生型或其突變體),通過機(jī)械刺激測定小鼠曲爪反應(yīng)疼痛閾值,進(jìn)行統(tǒng)計(jì)分析,最終確定樣品是否引起小鼠痛覺過敏。
6-1、短期致痛情況觀測
一、實(shí)驗(yàn)材料
動(dòng)態(tài)足底觸覺儀,意大利/ugobasile,型號37450。
二、實(shí)驗(yàn)內(nèi)容
1、合格小鼠篩選
訂購spf級別cd-1小鼠,小鼠規(guī)格為雄性,體重30-35g。
使用動(dòng)態(tài)足底觸覺儀[意大利/ugobasile,型號37450]篩選兩足平均閾值在7.5-10之間且同一只小鼠左右腳閾值p值,p值>0.05的實(shí)驗(yàn)動(dòng)物為合格小鼠。
隨機(jī)分組,分為實(shí)驗(yàn)組和空白對照組,實(shí)驗(yàn)組又按照不同樣品及給藥劑量分為小組,每組10只。
2、ngf樣品給藥設(shè)計(jì)
2.1野生型樣品致痛劑量篩選
藥物配制:將陽性對照ngf野生型樣品、ngfs47a突變體、ngfi31n突變體樣品均使用樣品儲(chǔ)備液(50mmpb,150mmnacl,ph6.8)進(jìn)行稀釋。
空白對照:為ngf樣品儲(chǔ)備液。
給藥方式及劑量:小鼠左右腳足底皮下分別給藥20ul;
短期給藥最低劑量為1.25μg/只。
3、痛閾測定
在給藥后分別在1個(gè)小時(shí)、2個(gè)小時(shí)進(jìn)行機(jī)械閾值測定,并且記錄數(shù)值,進(jìn)行短期給藥(2小時(shí)內(nèi))的觀測。
4、結(jié)果統(tǒng)計(jì)分析
采用graphpadprism軟件進(jìn)行圖表繪制及結(jié)果統(tǒng)計(jì)分析,比較各劑量組與對照組之間機(jī)械閾值是否有差異,分析野生型樣品以及突變體樣品的致痛性。
由圖2可知,給藥最低劑量為1.25μg/只時(shí),對照組小鼠無疼痛表現(xiàn),而野生型的陽性對照組,疼痛閾值在1h明顯低于5以下,疼痛明顯;而各突變體實(shí)驗(yàn)組疼痛閾值均在6至7左右,與陽性對照組相比,疼痛明顯減輕。
6-2長期致痛情況觀測
除設(shè)定本次實(shí)驗(yàn)的3個(gè)給藥劑量為:0.2μg/只、0.5μg/只、1.25μg/只,每天給藥一次,連續(xù)3周,期間不間斷的進(jìn)行疼痛閾值的測定外,其余均按照6-1“短期致痛情況觀測”中的方法進(jìn)行。
結(jié)果如圖3(a)和3(b)所示,表明:野生型ngf在17天以內(nèi)三個(gè)劑量顯示痛閾逐漸降低,逐漸出現(xiàn)疼痛感。s47a突變體、i31n突變體中低高三個(gè)劑量組的實(shí)驗(yàn)結(jié)果顯示在14天內(nèi)除s47a突變體高劑量組痛閾明顯降低引起疼痛外,其余劑量組痛閾基本都維持在6以上,與野生型相比有明顯減輕疼痛的作用。
實(shí)施例7.野生型hngf及其突變體是否致痛的行為學(xué)檢測
野生型hngf及其突變體樣品對小鼠關(guān)節(jié)給藥,通過行為學(xué)觀察小鼠抬腿時(shí)間以及次數(shù)來考察樣品是否致痛。
實(shí)驗(yàn)內(nèi)容
1、小鼠訂購
訂購spf級別cd-1小鼠,小鼠規(guī)格為雄性,體重30-35g。隨機(jī)分組,分為實(shí)驗(yàn)組、空白對照組(簡稱對照組)及陽性對照組,各組根據(jù)劑量又分為三個(gè)小組,每個(gè)小組隨機(jī)挑選6只。
2、給藥劑量與時(shí)間
2.1給藥劑量
陽性對照:采用樣品儲(chǔ)備液(50mmpb,150mmnacl,ph6.8)稀釋野生型hngf為1.25μg/10ul;
實(shí)驗(yàn)組:突變體藥物配制方法同陽性對照,稀釋ngfi31n突變體為1.25μg/10ul;
空白對照:生理鹽水。
2.2給藥方式
在小鼠后腿關(guān)節(jié)內(nèi)注射藥品,每個(gè)關(guān)節(jié)腔給藥10ul。
2.3給藥時(shí)間:
每劑量組進(jìn)行單次連續(xù)3-4天給藥。即第一天上午10點(diǎn),以及之后第2、3、4天相同時(shí)間點(diǎn)給藥。
3、行為學(xué)觀察
各實(shí)驗(yàn)組以及對照組當(dāng)天給藥后2、4小時(shí)進(jìn)行觀察,以及給藥的第2、3、4天相同時(shí)間點(diǎn)進(jìn)行觀察。
觀察指標(biāo):2分鐘內(nèi)小鼠自發(fā)性抬腿的次數(shù)以及每次抬腿維持的時(shí)間(秒),計(jì)算抬腿維持累計(jì)時(shí)間。
4、結(jié)果統(tǒng)計(jì)分析
采用graphpadprism軟件雙因素方差分析抬腿維持時(shí)間,比較分析不同樣品是否致痛。
實(shí)驗(yàn)結(jié)果如圖4所示:野生型hngf組在注射藥物后的各檢測時(shí)間點(diǎn)均能引起明顯的疼痛,i31n突變體實(shí)驗(yàn)組連續(xù)給藥疼痛現(xiàn)象不明顯,抬腿維持累計(jì)時(shí)間均在1秒以下卡方分析得知在各檢測時(shí)間點(diǎn)陽性對照組與實(shí)驗(yàn)組均有明顯差異,說明i31n突變體可明顯減輕藥物注射的疼痛反應(yīng)。
