背景技術(shù):
]

嵌合抗原受體(chimeric antigen receptor,CAR)T細(xì)胞治療(CAR-T)技術(shù)是近年來迅速發(fā)展起來的一種腫瘤過繼免疫治療的新手段。CAR是人工構(gòu)建的融合基因編碼的跨膜分子,由胞外區(qū)、胞內(nèi)區(qū)和跨膜區(qū)三部分構(gòu)成。胞外區(qū)負(fù)責(zé)腫瘤抗原的識別,胞內(nèi)區(qū)負(fù)責(zé)信號的轉(zhuǎn)導(dǎo),跨膜區(qū)連接胞外區(qū)和胞內(nèi)區(qū)。通過將識別腫瘤相關(guān)抗原的抗體的單鏈可變區(qū)(single-chain variable fragment,scFv)和胞內(nèi)信號域在體外進(jìn)行基因重組,生成重組質(zhì)粒,采用病毒包裝后再轉(zhuǎn)染至患者T細(xì)胞,使患者的T細(xì)胞表達(dá)腫瘤抗原受體,轉(zhuǎn)染后經(jīng)過純化和大量擴增后的T細(xì)胞,即為CART細(xì)胞。CAR能夠賦予T細(xì)胞HLA非依賴的方式識別腫瘤抗原的能力。目標(biāo)腫瘤抗原的選擇對于CAR的特異性、有效性以及基因改造T細(xì)胞自身的安全性都是關(guān)鍵的決定因素。目前,盡管識別CD19靶點的CART可以成功治愈B淋巴細(xì)胞白血病和B細(xì)胞淋巴瘤,但仍然存在治療失敗的病例,根本原因是對于有的B淋巴細(xì)胞白血病和B細(xì)胞淋巴瘤患者,CD19作為靶點的特異性不強,使CAR-T細(xì)胞不能正確識別腫瘤細(xì)胞。因此,開發(fā)多種腫瘤抗原組合的CAR,根據(jù)患者外周血中的腫瘤抗原的表達(dá)情況選擇CAR,制備的CAR-T細(xì)胞,通過多靶點識別,能提高對腫瘤細(xì)胞的識別能力,確保T細(xì)胞對腫瘤細(xì)胞的殺傷效率。

技術(shù)實現(xiàn)要素:
]

本發(fā)明提高T細(xì)胞對腫瘤細(xì)胞的識別和殺傷效率。

為了實現(xiàn)上述目的,提供一種多靶點聯(lián)合的CAR-T細(xì)胞制劑及其制備方法,由以下步驟構(gòu)成:

i.采用基因工程技術(shù)設(shè)計多個腫瘤抗原的組合,所述的腫瘤抗原的組合包括a.CD19;以及b.CD20、CD138、CD123中的一種或兩種,

ii.將腫瘤抗原的組合插入到載體序列的4513和4514位置之間構(gòu)成CAR,載體序列的堿基序列如SEQ ID NO.1所示;

iii.將CAR進(jìn)行病毒包裝;

iv.將CAR通過病毒轉(zhuǎn)染T細(xì)胞;

v.通過培養(yǎng)基進(jìn)行培養(yǎng);

vi.將制備的CAR-T細(xì)胞與人血白蛋白、生理鹽水混合,其中CAR-T細(xì)胞濃度為0.5~1×106個/毫升,CAR陽性率在25%以上,活率在95%以上。

包括兩種不同的CD123序列。

所述的腫瘤抗原組合為CD19-CD20、CD19-CD138、CD19-CD123、CD19-CD20-CD138、CD19-CD20-CD123、CD19-CD138-CD123。

本發(fā)明同現(xiàn)有技術(shù)相比,通過建立多種不同腫瘤抗原組合的CAR-T細(xì)胞以及多靶點識別,能提高對腫瘤細(xì)胞的識別能力,確保T細(xì)胞對腫瘤細(xì)胞的殺傷效率。

[具體實施方式]

以下,結(jié)合實施例對于本發(fā)明做進(jìn)一步說明,應(yīng)當(dāng)理解實施例僅用于解釋說明而不用于限定發(fā)明的保護(hù)范圍。

本實施例中的CAR-T細(xì)胞制劑的制備通過以下步驟實現(xiàn):

a.采用基因工程技術(shù)設(shè)計多個腫瘤抗原的組合,這些抗原涉及CD19、CD20、CD138、CD123,抗原組合的方式是CD19加上另外一個抗原組合和CD19加上另外二個抗原組合,即CD19-CD20,CD19-CD138,CD19-CD123,CD19-CD20-CD138,CD19-CD20-CD123,CD19-CD138-CD123,這是根據(jù)患者外周血中的腫瘤抗原的表達(dá)而選擇的,CD19、CD20、CD138、CD123的基因?qū)τ诒绢I(lǐng)域的技術(shù)人員是清楚的,如

CD19可選擇兩個序列,一條為鼠源的(CD19-1,如SEQID NO.2所示),一條為人源的(CD19-2,如SEQ ID NO.3所示),因此,CD19的選擇可以是上述兩個中的任意一個;

CD20的堿基序列如SEQ ID NO.4所示。

CD123有兩個序列,標(biāo)記為CD123-1(如SEQ ID NO.5所示),CD123-2(如SEQ ID NO.6所示),CD123的選擇可以是上述兩個中的任意一個。

CD20的堿基序列如SEQ ID NO.7所示。

b.載體的序列為CARVec,如SEQ ID NO.1所示,將上面a.組合的序列插入到載體序列的4513和4514位置之間;

c.將CAR進(jìn)行病毒包裝;

d.將CAR通過病毒轉(zhuǎn)染T細(xì)胞;

e.通過培養(yǎng)基進(jìn)行培養(yǎng);

f.將制備的CAR-T細(xì)胞與人血白蛋白、生理鹽水混合,其中CAR-T細(xì)胞濃度為0.5~1×106個/毫升,CAR陽性率在25%以上,活率在95%以上。
